These Terms of Service (“Terms”) constitute a binding legal agreement between you and Integrity Insure (“Company,” “we,” “us,” or “our”) governing your access to and use of integrityhealth.care, including all related pages, tools, forms, and services (collectively, the “Site”).

By accessing or using the Site in any manner, you confirm that you have read, understood, and agree to be bound by these Terms, our Privacy Policy, and any additional terms or notices posted on the Site. If you do not agree to these Terms, you must discontinue use of the Site immediately.

These Terms apply to all visitors, users, and any other persons who access or use the Site, including those who submit quote requests, contact forms, or schedule consultations through the Site.

If you are using the Site on behalf of a business or organization, you represent that you have the authority to bind that entity to these Terms, and “you” will refer to both you individually and that entity.

We may use third-party analytics tools — such as Google Analytics — to understand Site traffic, page performance, visitor behavior, and usability. These tools use cookies and similar technologies to collect data about how you interact with the Site.

  • Consultation scheduling tools to connect visitors with Robert Colucci, our licensed advisor
  • State-specific coverage information and eligibility education
  • A contact channel for general insurance inquiries

Integrity Insure is currently licensed to operate in 30+ states across the United States. Coverage availability, plan options, and eligibility rules vary significantly by state, carrier, and individual circumstances. Nothing on the Site constitutes a guarantee of plan availability, pricing, or eligibility in any specific state.

We operate as an independent advisory and are not affiliated with, endorsed by, or agents of any specific insurance carrier. Recommendations are made based on the visitor’s stated needs, budget, and eligibility — not on carrier incentive structures.

The content on integrityhealth.care is provided for general informational and educational purposes only. It does not constitute legal advice, tax advice, financial planning advice, or medical advice. No advisor-client, attorney-client, or fiduciary relationship is created by your use of this Site or submission of a quote request.

Insurance product information, plan comparisons, cost estimates, and eligibility explanations on the Site are provided as educational guidance only. Actual plan availability, premiums, deductibles, networks, and benefits are determined by the relevant insurance carrier and may differ from illustrative examples.

Tax treatment of insurance premiums, health savings accounts, employer contributions, and related topics can vary significantly based on individual circumstances. Intellectual Property Rights

Unless otherwise stated, the Site and all of its content — including but not limited to text, articles, guides, page layouts, design elements, logos, graphics, icons, color schemes, and other materials — are owned by or licensed to Integrity Insure and are protected by applicable copyright, trademark, and intellectual property laws.

You are granted a limited, non-exclusive, non-transferable, revocable license to access and use the Site solely for your personal, non-commercial informational purposes. This license does not include the right to:

  • Reproduce, copy, republish, or redistribute any Site content without prior written permission
  • Use any Site content for commercial purposes or to create competing services
  • Modify, adapt, translate, or create derivative works based on Site content
  • Remove or alter any copyright, trademark, or proprietary notices on the Site
  • Frame or mirror the Site on any other website or application

User Representations

By accessing and using the Site, you represent and warrant that:

  • You are at least 18 years of age, or a legal guardian acting on behalf of a household member
  • You have the legal capacity and authority to enter into these Terms and be bound by them
  • All information you submit through the Site — including quote requests, contact forms, and scheduling tools — is accurate, truthful, and complete to the best of your knowledge
  • You will not impersonate or misrepresent your identity or affiliation with any person or organization
  • Your use of the Site will comply with all applicable federal, state, and local laws and regulations
  • You will not attempt to access or use the Site through automated means (bots, scrapers, scripts) without our prior written authorization
  • You are not subject to any government sanctions or located in a jurisdiction where use of this Site would be prohibited

If any representation becomes inaccurate after you begin using the Site, you agree to promptly discontinue use and, where applicable, notify us at robert@integrityhealth.care.

Prohibited Activities

You agree that you will not use the Site to engage in any of the following prohibited activities:

  • Violating any applicable law, regulation, rule, or ordinance — federal, state, or local
  • Submitting false, misleading, or fraudulent information through any form or inquiry
  • Attempting unauthorized access to any portion of the Site, its underlying systems, servers, or associated databases
  • Transmitting or uploading malware, viruses, spyware, trojans, ransomware, or any other harmful or malicious code
  • Engaging in scraping, crawling, data harvesting, or systematic extraction of Site content without written permission
  • Using the Site to send unsolicited communications (spam) to us or others
  • Interfering with or disrupting the integrity, performance, or availability of the Site or its supporting infrastructure
  • Attempting to reverse engineer, decompile, or disassemble any portion of the Site
  • Circumventing or attempting to bypass any security, rate-limiting, or abuse-prevention measures we have implemented
  • Using the Site for any purpose that competes with or harms Integrity Insure’s business
  • Impersonating another person, entity, or agent in any communication through the Site

Violation of these prohibitions may result in immediate termination of your access to the Site, and may be referred to appropriate law enforcement or regulatory authorities where warranted.

Quote & Form Submissions

By submitting a quote request, contact form, or scheduling request through the Site, you authorize Integrity Insure to use the information you provide to:

  • Research available insurance plans that may fit your stated needs and eligibility
  • Contact you by email, phone, or text message to discuss your inquiry and follow up on options
  • Share your information with licensed carriers and wholesale partners as required to deliver quotes, as described in our Privacy Policy

Submitting a quote request does not obligate you to purchase any insurance product, enroll in any plan, or enter into any agreement. Likewise, it does not obligate Integrity Insure to guarantee plan availability, pricing, or enrollment outcome in any specific scenario.

You agree to submit only accurate, truthful information through our forms. Submitting intentionally false information — including falsified identities, fraudulent income or household data, or misrepresented health histories — may constitute fraud under applicable state and federal law and may be reported to appropriate authorities.

We do not sell quote submission data to third-party marketers, lead aggregators, or data brokers. Information submitted through our quote funnel is used exclusively to serve your insurance inquiry.

§9

Contributions & Submissions

If you submit comments, feedback, testimonials, ideas, suggestions, or other content to us through the Site or by email, you grant Integrity Insure a non-exclusive, perpetual, royalty-free, worldwide license to use, reproduce, modify, adapt, and publish that content for any lawful business purpose — including testimonial display, service improvement, or marketing — without compensation to you, unless otherwise required by applicable law.

You represent that any content you submit does not infringe on the rights of any third party (including copyright, trademark, privacy, or defamation rights), is not false or misleading, and does not violate any applicable law or regulation.

You remain solely responsible for the accuracy, legality, and appropriateness of any content you submit. We reserve the right to remove, edit, or decline to publish any submission at our discretion.

The Site may contain links to external websites, insurance carrier portals, government resources (such as Healthcare.gov), scheduling tools, or other third-party services. These links are provided for your convenience and informational purposes only.

Integrity Insure does not endorse, control, or assume responsibility for the content, accuracy, privacy practices, or availability of any third-party website or service. Your use of any external site is subject to that site’s own terms of service and privacy policy.

The presence of a link on the Site does not imply a partnership, affiliation, sponsorship, or endorsement of any kind. We encourage you to review the terms and privacy policies of any external sites you visit through links on our Site.

Site Management

We reserve the right, but not the obligation, to:

  • Monitor the Site for violations of these Terms or misuse of our services
  • Take any legal or operational action we deem appropriate in response to suspected violations
  • Restrict, suspend, or terminate access for any user who violates these Terms, submits fraudulent information, or otherwise misuses the Site
  • Remove or refuse to display any user-submitted content that we determine, in our sole discretion, is inappropriate, inaccurate, harmful, or in violation of these Terms
  • Implement technical measures to prevent abuse, including rate limiting, IP blocking, and bot detection
  • Cooperate with law enforcement, regulators, or other authorities when legally required or when we determine cooperation is appropriate

These rights are exercised at our sole discretion and do not create an obligation on our part to monitor, detect, or act against any particular violation at any particular time.

Your use of the Site is also governed by our Privacy Policy, which is incorporated into these Terms by this reference. The Privacy Policy explains how we collect, use, store, and protect personal information you submit through the Site — including quote requests, contact forms, and scheduling interactions.

  • California residents have additional rights under the CCPA and CPRA

§13

Term & Termination

These Terms remain in full force and effect for as long as you access or use the Site. You may stop using the Site at any time.

We may, at our sole discretion, restrict, suspend, or permanently terminate your access to the Site — with or without notice — for any reason, including but not limited to:

  • Violation of any provision of these Terms
  • Submission of fraudulent, false, or misleading information
  • Abuse of our quote, contact, or scheduling systems
  • Behavior that is harmful to other users, our business, or third parties
  • Legal, regulatory, or operational necessity

Upon termination, your right to use the Site ceases immediately. Provisions of these Terms that by their nature should survive termination — including intellectual property rights, disclaimers, limitations of liability, and indemnification — will continue to apply after termination.

§14

Modifications & Interruptions

We reserve the right to modify, update, suspend, or discontinue the Site or any part of it — including specific pages, tools, forms, or services — at any time, with or without prior notice. This includes the right to modify these Terms at any time by posting a revised version on the Site with an updated “Last Updated” date.

Your continued use of the Site after any modification to these Terms constitutes your acceptance of the revised Terms. If you do not agree to the modified Terms, you must stop using the Site.

We do not guarantee uninterrupted, error-free availability of the Site. We may experience downtime due to maintenance, technical issues, third-party outages, or circumstances outside our control. We are not liable for any loss, damage, or inconvenience resulting from Site unavailability or service interruptions.

§15

Governing Law

These Terms and any dispute arising out of or related to your use of the Site shall be governed by and construed in accordance with the laws of the State of New York, without regard to its conflict of law principles.

To the extent any applicable federal law governs, such federal law shall apply where it supersedes or supplements state law. Nothing in this section limits rights you may have under mandatory consumer protection laws of your state of residence.

Dispute Resolution

Before initiating any formal legal claim or proceeding, you agree to first attempt good-faith informal resolution by contacting us at robert@integrityhealth.care with a written description of the dispute, including the nature of the issue, the relevant dates, and the resolution you are seeking. We will make a good-faith effort to resolve the matter within 30 calendar days of receipt.

If the dispute cannot be resolved informally within that period, either party may pursue any available legal remedies. Any formal dispute or claim arising under these Terms may be brought in courts of competent jurisdiction located in the State of New York, and both parties consent to personal jurisdiction in those courts — unless applicable law requires otherwise.

Nothing in this section prevents either party from seeking emergency injunctive or other equitable relief in any court of competent jurisdiction to prevent irreparable harm while informal resolution or formal proceedings are pending.

Disclaimer of Warranties

THE SITE AND ALL CONTENT, TOOLS, FORMS, AND SERVICES ARE PROVIDED ON AN “AS IS” AND “AS AVAILABLE” BASIS WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE, AND NON-INFRINGEMENT.

We make no warranty that:

  • The Site will be available continuously, uninterrupted, or error-free
  • Information on the Site is complete, accurate, current, or free from inaccuracies or omissions
  • Any specific insurance plan, premium, network, or benefit described on the Site will be available to you
  • Results obtained from using the Site or submitting a quote request will meet your expectations or requirements
  • The Site or its servers are free from viruses, malware, or other harmful components

Insurance product availability, pricing, benefits, deductibles, network access, and eligibility criteria are established by insurance carriers and are subject to change at any time. Final plan details are always governed by the carrier’s official plan documents, not by any representation on this Site.

§18

Limitations of Liability

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, INTEGRITY INSURE, ITS OFFICERS, DIRECTORS, EMPLOYEES, ADVISORS, AGENTS, LICENSORS, AND SERVICE PROVIDERS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATED TO YOUR ACCESS TO OR USE OF THE SITE — INCLUDING BUT NOT LIMITED TO LOSS OF REVENUE, LOSS OF DATA, LOSS OF GOODWILL, BUSINESS INTERRUPTION, OR COST OF SUBSTITUTE SERVICES — EVEN IF WE HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.

IN JURISDICTIONS THAT DO NOT ALLOW THE EXCLUSION OR LIMITATION OF INCIDENTAL OR CONSEQUENTIAL DAMAGES, THE ABOVE LIMITATIONS APPLY ONLY TO THE FULLEST EXTENT PERMITTED BY LAW.

TO THE EXTENT DIRECT DAMAGES ARE PERMITTED, OUR TOTAL AGGREGATE LIABILITY TO YOU FOR ANY CLAIM ARISING UNDER THESE TERMS OR RELATED TO USE OF THE SITE SHALL NOT EXCEED THE GREATER OF: (A) THE AMOUNT YOU PAID TO INTEGRITY INSURE IN THE TWELVE MONTHS PRECEDING THE CLAIM (IF ANY), OR (B) ONE HUNDRED U.S. DOLLARS ($100).

Nothing in this section limits liability for death or personal injury caused by gross negligence, for fraud or intentional misconduct, or for any liability that cannot be excluded or limited under applicable law.

§19

Indemnification

You agree to defend, indemnify, and hold harmless Integrity Insure and its officers, directors, employees, agents, licensors, and service providers from and against any and all claims, liabilities, damages, judgments, awards, losses, costs, expenses, and fees (including reasonable attorneys’ fees) arising out of or relating to:

  • Your violation of these Terms or any applicable law or regulation
  • Your submission of false, fraudulent, or inaccurate information through the Site
  • Your misuse of the Site or any of its tools, forms, or services
  • Any content you submit, post, or transmit through the Site
  • Any infringement by you of the rights of a third party

We reserve the right to assume exclusive control of the defense of any matter subject to indemnification by you, in which case you agree to cooperate fully with us in asserting any available defenses at your expense.

§20

Miscellaneous

Entire Agreement. These Terms, together with our Privacy Policy and any other legal notices or policies posted on the Site, constitute the entire agreement between you and Integrity Insure regarding your use of the Site and supersede all prior agreements, understandings, representations, and warranties related to the same.

Severability. If any provision of these Terms is found to be invalid, illegal, or unenforceable by a court of competent jurisdiction, that provision will be modified to the minimum extent necessary to make it enforceable, or severed if modification is not possible, and the remaining provisions will continue in full force and effect.

Waiver. Our failure to enforce any right or provision of these Terms will not be considered a waiver of that right or provision. No waiver of any breach by you shall be deemed a waiver of any subsequent breach.

No Assignment. You may not assign or transfer any of your rights or obligations under these Terms without our prior written consent. We may assign these Terms, in whole or in part, without restriction.

Headings. Section headings in these Terms are for organizational reference only and do not affect the interpretation or meaning of any provision.

Electronic Communications. By using the Site or submitting forms, you consent to receive communications from us electronically. You agree that all disclosures, agreements, notices, and other communications we provide electronically satisfy any legal requirement that such communications be in writing.
