§6
Quote & Form Submissions
When you submit a quote request through integrityhealth.care, we collect the information you provide — including personal details, household information, and coverage preferences — to understand your insurance needs and identify available plan options in your area.
Depending on your coverage type and state of residence, quote request information may be shared with licensed insurance carriers, wholesale brokers, and distribution partners as necessary to research and present suitable options. We share only the minimum information required to facilitate this process, and only with parties authorized to use it for that purpose.
Our quote and contact forms include technical anti-abuse protections — including IP-based rate limiting, submission timing analysis, and automated spam filtering. These safeguards protect the integrity of submitted data and prevent misuse of our service infrastructure. They are entirely transparent to legitimate users and do not affect normal form usage.
All form submissions are transmitted over encrypted connections (HTTPS). Quote data is not intercepted or logged in unprotected formats.
If you submit a request and do not receive a response within one business day, please check your spam folder or contact us directly at robert@integrityhealth.care.
§7
SMS / Text Messaging
If you provide a mobile phone number as part of a quote request, consultation booking, or other service inquiry, we may contact you via text message with relevant quote updates, appointment reminders, or service-related communications.
Message frequency varies based on the nature of your inquiry and the stage of your advisory engagement. Standard message and data rates may apply depending on your mobile carrier plan.
You may opt out of text messaging at any time by:
- Replying STOP to any message we send you
- Emailing robert@integrityhealth.care with the subject line “Opt Out — Text Messaging”
We do not use your phone number for automated marketing campaigns, bulk SMS broadcasts, or any purpose unrelated to your direct service inquiry or active advisory relationship.
§8
Third-Party Insurance Carriers & Partners
In order to research and deliver insurance plan options on your behalf, we may share relevant personal information with the following categories of third parties:
- Licensed insurance carriers authorized to offer individual, family, or group plans in your state
- Wholesale insurance brokers and distribution partners who facilitate plan access and carrier quoting
- Enrollment and plan-comparison platform providers used to research and present coverage options
- Service providers supporting our operations — including email delivery, analytics, hosting, and scheduling tools
Each of these parties receives only the information necessary to provide the services you have requested. We do not authorize them to use your information for their own independent marketing purposes, to build profiles for advertising, or to resell your data without restriction.
We are not responsible for the privacy practices of third-party websites or services linked from integrityhealth.care. We encourage you to review the privacy policies of any external sites or carriers you engage with directly.
§9
Retention & Transfer of Information
We retain personal information for as long as reasonably necessary to provide services, fulfill our legal and regulatory obligations, resolve disputes, and enforce our agreements. Specific retention periods may vary depending on the type of information and applicable requirements.
When you request deletion of your personal information (see §13 — Your Privacy Rights), we will process your request promptly and remove or appropriately anonymize data not required to be retained by law or regulation.
Our systems and third-party service providers may process and store data in the United States. By using our services, you understand and consent to applicable data processing and storage within the United States, where data protection standards may differ from those in your jurisdiction.
§10

We use commercially reasonable administrative, technical, and organizational safeguards to protect personal information from unauthorized access, disclosure, alteration, use, or destruction. Our security practices include:
- HTTPS encryption for all data transmitted through our website and form endpoints
- Access controls limiting which personnel or systems can view or process personal information
- Anti-abuse protections on all form submission endpoints, including rate limiting, honeypot fields, and timing analysis
- Server-side validation and sanitization of all submitted data to prevent injection and manipulation attacks
- Regular review of data handling practices and third-party service provider agreements
No method of transmission over the internet or electronic storage is completely secure. While we work diligently to protect your information, we cannot guarantee absolute security against all possible threats. In the event of a data breach that materially affects your personal information, we will take appropriate notification and remediation steps as required by applicable law.
§12
Health Information & Regulatory Notes
We may collect limited health-related or health-status information as part of the insurance quoting and guidance process. This may include, for example, tobacco use status, general health status indicators, prescription drug categories, or whether you have a qualifying life event. This information is used solely to identify insurance plan options appropriate for your situation and is not stored beyond what is required for service delivery.
If you believe your situation requires specialized compliance handling beyond standard insurance quoting and advisory workflows, please contact us before sharing sensitive health information so we can discuss the appropriate channel.
